Air traffic control was introduced in reaction to fatal collisions. But satellite orbits are not like air traffic routes. When impacts occur in the air, gravity will immediately clean up the sky: airplane parts fall to the ground. In space, however, a swarm of pieces of different dimensions, will continue to orbit within the same belt for a long time.
